Application of bone flap-preserving decompressive craniotomy in patients with elevated intracranial pressure

Abstract:Many neurosurgical critical conditions,such as severe traumatic brain injury and intracerebral hemorrhage,are prone to causing elevated intracranial pressure(ICP).The level of ICP is a crucial factor influencing patient survival and prognosis.Controlling ICP stands as the primary goal in the treatment of neurosurgical critical illnesses.Building upon traditional decompressive craniectomy,some scholars have attempted bone flap preservation techniques,developing approaches like floating craniotomy and hinged craniotomy.These methods not only reduce ICP but also offer advantages such as fewer complications,avoidance of subsequent cranioplasty,and lower surgical costs,presenting a potential alternative to decompressive craniectomy.This article reviews the indications,surgical techniques,efficacy,and complications of bone flap-preserving decompressive craniotomy,aiming to provide references for surgical decompression in patients with elevated ICP.
