Correlation between serum sCD40L,sFlt-1,and hemorrhagic transformation after intravenous thrombolysis in patients with acute ischemic stroke

Abstract:Objective To investigate the correlation between serum soluble CD40 ligand(sCD40L),soluble vascular endothelial growth factor receptor-1(sFlt-1),and hemorrhagic transformation(HT)after intravenous thrombolysis in patients with acute ischemic stroke(AIS).Methods A total of 136 AIS patients prospectively admitted from January 2020 to January 2023 were enrolled.All received intravenous thrombolysis.Head CT was repeated within 7 days to diagnose HT.Additionally,136 healthy individuals undergoing physical examinations were selected as healthy controls.Serum levels of sCD40L and sFlt-1 were measured using enzyme-linked immunosorbent assay upon admission.Results Serum levels of sCD40L and sFlt-1 were significantly higher in AIS patients compared to the control group(P<0.05).Among the 136 AIS patients,26(19.1%)developed HT.Multivariate logistic regression analysis revealed that elevated serum sCD40L level(OR=2.253,95%CI 1.189~4.268,P=0.013)and elevated serum sFlt-1 level(OR=1.982,95%CI 1.109~3.540,P=0.028)were independent risk factors for HT after intravenous thrombolysis in AIS patients.ROC curve analysis showed that serum sCD40L≥5.66 ng/mL and serum sFlt-1≥85.24 pg/mL had certain predictive value for HT after intravenous thrombolysis in AIS patients.Their combination demonstrated better predictive performance(AUC=0.903,95%CI 0.841~0.947,sensitivity 75.92%,specificity 93.64%).Conclusion Serum levels of sCD40L and sFlt-1 are significantly elevated in AIS patients.The combination of these two biomarkers shows good predictive value for HT after intravenous thrombolysis in AIS patients.
