Application of self-stopping drills versus burr drills for hole creation in burr hole drainage for chronic subdural hematoma

Abstract:Objective To compare the application effects of self-stopping drills versus burr drills for hole creation in burr hole drainage for chronic subdural hematoma.Methods The clinical data of 80 patients with chronic subdural hematoma treated by burr hole drainage from January 2019 to December 2022 were retrospectively analyzed.Among them,40 patients underwent hole creation using a self-stopping drill(self-stopping drill group),and 40 patients underwent hole creation using a burr drill(burr drill group).Results Compared with the burr drill group,the self-stopping drill group showed a significantly higher incidence of bleeding around the bone hole during drilling(P<0.05),significantly higher patient pain scores during drilling(P<0.05),and a significantly higher postoperative drainage tube deviation rate(P<0.05).There were no statistically significant differences between the two groups in operative time,hematoma clearance rate,hospital stay,postoperative complication rate,or postoperative hematoma recurrence rate(P>0.05).Conclusion Using a burr drill for hole creation in burr hole drainage for chronic subdural hematoma can reduce bleeding around the bone hole,alleviate patient discomfort during drilling,and decrease the postoperative drainage tube deviation rate,without increasing operative time.It is an effective drilling technique.
