Efficacy of middle meningeal artery embolization for patients with chronic subdural hematoma

Abstract:Objective To investigate the clinical efficacy of middle meningeal artery embolization in the treatment of chronic subdural hematoma(CSDH).Methods The clinical data of 113 patients with CSDH admitted between January 1,2021,and September 30,2024,were retrospectively analyzed.Among them,38 patients were treated with middle meningeal artery embolization(embolization group),and 75 patients were treated with oral atorvastatin(conservative group).To reduce selection bias,propensity score matching(PSM)analysis was performed for 1:1 matching.Outcome measures included treatment failure(clinical deterioration requiring surgical hematoma evacuation),changes in hematoma thickness and midline shift at 1 and 3 months of follow-up,and poor prognosis(modified Rankin Scale score>2)at 3 months of follow-up.Results After PSM analysis,25 patients were included in the embolization group and 25 in the conservative group.The treatment failure rate in the embolization group(4.00%)was significantly lower than that in the conservative group(28.00%;P=0.049).At the 1-and 3-month follow-ups,hematoma thickness and midline shift were significantly improved in the embolization group compared to the conservative group(P<0.05).At the 3-month follow-up,the rate of poor prognosis in the embolization group(0%)was significantly lower than that in the conservative group(20.00%;P=0.050).Conclusion Middle meningeal artery embolization appears to be effective and safe for the treatment of CSDH.
