Efficacy of thrombectomy via the posterior communicating artery for acute internal carotid artery occlusion combined with posterior communicating artery occlusion

Abstract:Objective To investigate the efficacy and safety of thrombectomy via the posterior communicating artery(PComA)for the treatment of acute internal carotid artery(ICA)occlusion combined with PComA occlusion.Methods The clinical data of 5 patients with acute ICA occlusion combined with PComA occlusion,admitted between February 2021 and February 2024,were retrospectively analyzed.Results All 5 patients underwent stent thrombectomy via the PComA,achieving successful recanalization with a Thrombolysis in Cerebral Infarction(TICI)grade of 3.The National Institutes of Health Stroke Scale(NIHSS)score one week after surgery[(5.8±4.9)]was significantly lower than the preoperative score[(17.4±5.1);P=0.011].One case was complicated by rupture of the posterior communicating artery due to traction,resulting in subarachnoid hemorrhage,with a modified Rankin Scale(mRS)score of 4 at the 3-month follow-up;one case developed pulmonary infection and myocardial infarction postoperatively,with an mRS score of 5 at the 3-month follow-up.Three cases had no complications,among whom two had an mRS score of 1 and one had a score of 2 at the 3-month follow-up.No new strokes or re-occlusion events occurred during the 3-month follow-up period.Conclusion For acute ICA occlusion combined with PComA occlusion,thrombectomy via the PComA is a safe and effective treatment method.
