Relationship between gut microbiota metabolism and prognosis in acute ischemic stroke patients

Abstract:Objective To investigate the relationship between alterations in gut microbiota and the prognosis of patients with acute ischemic stroke(AIS).Methods A total of 140 AIS patients were prospectively enrolled from January 2020 to June 2022,along with 60 healthy individuals as controls.Fecal bacterial populations were analyzed using 16S rRNA gene sequencing.At 90 days post-stroke,the prognosis of AIS patients was assessed using the modified Rankin Scale(mRS)score,with a score≥3 defined as poor prognosis.Results At 90 days post-stroke,33 patients(23.6%)had a poor prognosis.Analysis at the phylum level revealed that AIS patients showed decreasing trends in the relative abundances of Firmicutes and Bacteroidetes and increasing trends in Actinobacteria and Proteobacteria,although these differences were not statistically significant(P>0.05).However,the relative abundances of Verrucomicrobia and Synergistetes were significantly increased in AIS patients(P<0.05).At the genus level,AIS patients exhibited a significant increase in the relative abundance of Megamonas(P<0.05)and a significant decrease in Agathobacter(P<0.05).At the genus level,*Ruminococcaceae UCG-002*(r=0.524,P<0.05),*Ruminococcaceae UCG-014*(r=0.499,P<0.05),and f_Christensenellaceae(r=0.446,P<0.05)showed significant positive correlations with the 90-day mRS score.Conclusion Patients with AIS exhibit structural changes in their gut microbiota.*Ruminococcaceae UCG-014*,*Ruminococcaceae UCG-002*,and f_Christensenellaceae may serve as predictive markers for early poor prognosis in AIS patients.
