Relationship between cognitive impairment and white matter tracts injury after basal ganglia cerebral hemorrhage

Abstract:Objective To investigate the relationship between white matter tract injury and cognitive dysfunction following hypertensive basal ganglia hemorrhage(HBGH).Methods A total of 24 patients with HBGH were prospectively enrolled from September 2019 to September 2023,along with 17 healthy subjects as controls.Diffusion tensor imaging(DTI)was performed within 1~3 weeks after hemorrhage.DTI raw data were imported into DSI-Studio software for three-dimensional reconstruction of cognition-related white matter tracts,including the superior longitudinal fasciculus,uncinate fasciculus,cingulum,inferior longitudinal fasciculus,and hippocampus.Cognitive function was assessed using the Mini-Mental State Examination(MMSE)and Montreal Cognitive Assessment(MoCA)at 3 weeks and 3 months post-hemorrhage.Results Compared with healthy subjects,HBGH patients showed significantly reduced fiber tracts in the superior longitudinal fasciculus,uncinate fasciculus,cingulum,and inferior longitudinal fasciculus(P<0.05),but no significant change in the hippocampus(P>0.05).Compared with the unaffected side of HBGH patients,the affected side exhibited significantly reduced fiber tracts in the inferior longitudinal fasciculus and uncinate fasciculus(P<0.05).At both 3 weeks and 3 months post-hemorrhage,MMSE and MoCA scores were significantly lower in patients than in controls(P<0.05).Conclusion Cognitive dysfunction in patients with HBGH may be associated with injuries to fiber tracts such as the superior longitudinal fasciculus,inferior longitudinal fasciculus,uncinate fasciculus,and cingulum.This suggests that the extent of damage to these tracts may serve as an indicator for evaluating cognitive prognosis in HBGH patients,providing a basis for treatment planning and outcome assessment.
