Value of brain volume and cisternal blood volume in the prognostic assessment of patients with aneurysmal subarachnoid hemorrhage

Abstract:Objective To investigate the value of brain volume and cisternal blood volume in the prognostic assessment of patients with aneurysmal subarachnoid hemorrhage(aSAH).Methods The clinical data of 160 aSAH patients admitted between January 2019 and January 2021 were retrospectively analyzed.Brain volume and cisternal blood volume were measured using 3D Slicer software based on dynamic CT imaging within one week post-admission.The brain volume change rate was calculated as(maximum brain volume-minimum brain volume)/cranial cavity volume×100%.At the last follow-up,prognosis was assessed using the Glasgow Outcome Scale(GOS)score:good prognosis(GOS 4~5)and poor prognosis(GOS 1~3).Multivariate logistic regression analyzed prognostic factors,while ROC curves evaluated the predictive value of brain volume and cisternal blood volume.Results The patients were followed for 9~24 months(median:14.6 months).Among 160 cases,98 had good prognoses and 62 had poor prognoses.Multivariate analysis identified a higher brain volume change rate(OR=3.456;95%CI 1.359~8.784;P=0.009)and interpeduncular cistern blood volume(OR=2.622;95%CI 1.240~5.544;P=0.012)as independent risk factors for poor prognosis.ROC analysis showed AUCs for predicting poor prognosis were 0.784(95%CI 0.711~0.857)for brain volume change rate and 0.712(95%CI 0.631~0.792)for interpeduncular cistern blood volume,with sensitivities of 77.42%and 66.13%,and specificities of 71.43%and 60.20%,respectively.Conclusion Brain volume change rate and interpeduncular cistern blood volume measured within one week post-admission hold prognostic value for aSAH patients.
