Multiple metastatic pleomorphic adenomas in the intracranial and spinal canal regions:a case report

Abstract:Pleomorphic adenoma is the most common benign tumor of the salivary glands.Despite its benign histology,it can exhibit local recurrence and metastasis.Metastasizing pleomorphic adenoma is a rare malignant salivary gland tumor that typically metastasizes to bone,lung,and cervical lymph nodes,with central nervous system involvement being exceedingly rare.This article reports a case of intracranial and intraspinal multifocal metastasizing pleomorphic adenoma in a 63-year-old male.The patient had undergone four prior resections for a right submandibular gland tumor two years earlier.Upon admission,cranial MRI and thoracic CT/MRI revealed space-occupying lesions in the left parieto-occipital lobe and at the T10 level of the spinal canal.The patient first underwent resection of the T10 intraspinal lesion,followed by resection of the left parieto-occipital lesion 24 days later.Postoperative pathological examination confirmed both lesions as pleomorphic adenoma.At the 4-year follow-up,the patient showed excellent recovery with complete resection of both intracranial and intraspinal lesions and no signs of recurrence.This case highlights that metastasizing pleomorphic adenoma should be included in the differential diagnosis when lesions involve both the spinal canal and intracranial compartment.Although surgical resection of metastasizing pleomorphic adenoma portends a favorable prognosis,close long-term follow-up remains essential.
