Treatment of large or giant basilar artery aneurysms

Abstract:Objective To investigate the clinical characteristics,treatment approaches and outcomes of large or giant basilar artery aneurysms.Methods A retrospective analysis was conducted on the clinical data of 10 patients with large or giant basilar artery aneurysms treated between August 2011 and January 2023.Results Stent-assisted coil embolization was performed in 8 cases.Immediate postoperative angiography revealed dense packing in 6 cases,and loose packing in 2 cases.Postoperative complications included water aspiration cough and left incomplete facial nerve palsy in 1 case,and bilateral mydriasis and decerebrate rigidity in 3 cases.Mortality outcomes included 3 deaths within 7 days postoperatively,and 1 death at 6 months postoperatively.Imaging follow-up(1~7 years)in the 4 surviving cases showed mid-aneurysm filling in 1 case,aneurysm neck recurrence in 1 case,no recurrence in 1 case,and undeterminable embolization status due to artifacts on CTA in 1 case.Clinical follow-up(1~12 years)in the 4 survivors showed a modified Rankin Scale(mRS)score of 1 in 2 cases,a mRS score of 2 in 1 case,and a mRS score of 4 in 1 case.Of 2 cases received conservative management,1 case died due to inability to feed at 1 year after discharge and 1 died at 2 years after discharge(unknown cause).Conclusions Large or giant basilar artery aneurysms have a poor prognosis when untreated.Stent-assisted coil embolization is associated with a relatively high complication rate.Severe postoperative complications occur more frequently in giant aneurysms involving the upper basilar segment with bilateral midbrain compression.
