Efficacy of interventional treatment via transradial artery approach for patients with intracranial aneurysms

Abstract:Objective To investigate the feasibility,safety,and efficacy of the transradial approach for the interventional treatment of intracranial aneurysms.Methods The clinical data of 29 patients with intracranial aneurysms treated with interventional treatment via the transradial approach between March 2023 and April 2024 were retrospectively analyzed.Results A total of 35 aneurysms were treated in the 29 patients,comprising 29 in the anterior circulation and 6 in the posterior circulation.According to the Myla classification of the aortic arch,5 cases were type I,10 were type II,and 14 were type III,including 3 cases of bovine arch.The transradial access was successfully established in 26 cases(89.7%,26/29),with 3 failures.One approach-related complication occurred(radial artery occlusion).Immediate postoperative angiography revealed Raymond-Roy Occlusion Classification(RROC)grade I occlusion in 24 aneurysms,grade II in 3,and grade III in 8.At the 3~6 month follow-up after discharge,all 29 patients had a modified Rankin Scale(mRS)score of 0.Angiographic follow-up at 3~6 months postoperatively for the 35 aneurysms showed RROC grade I occlusion in 31 aneurysms and grade II in 4.Conclusion Establishing access via the transradial artery for the interventional treatment of intracranial aneurysms is technically safe and feasible,with a low complication rate.
