Efficacy and safety for patients with unruptured intracranial posterior circulation aneurysms:flow diverters versus stent-assisted coiling

Abstract:Objective To compare the safety and efficacy of flow diverters versus stent-assisted coiling for patients with unruptured intracranial posterior circulation aneurysms.Methods The clinical data of 41 patients with unruptured intracranial posterior circulation aneurysms treated with endovascular intervention between January 1,2019,and January 1,2023,were retrospectively analyzed.Primary endpoints:Angiography at 1-year follow-up assessed aneurysm occlusion using the OKM scale;prognosis was evaluated via the modified Rankin Scale(mRS),with a score of 0~2 indicating good prognosis;perioperative complications were recorded.Results A total of 43 aneurysms were identified in 41 patients.Among these,13 patients(14 aneurysms)received flow diverters,and 28 patients(29 aneurysms)underwent stent-assisted coiling.At 1-year follow-up:the complete occlusion rate in the flow diverter group(78.6%)showed no statistical difference from the stent-assisted coiling group(75.9%;P=0.884);the good prognosis rate in the flow diverter group(92.3%)showed no statistical difference from the stent-assisted coiling group(92.9%;P>0.999);perioperative complications included 1 case of ischemic stroke in the flow diverter group,and 1 intracerebral hemorrhage,1 intraoperative thrombosis,and 1 intraoperative aneurysm rupture in the stent-assisted coiling group.The perioperative complication rate in the flow diverter group(7.7%)showed no statistical difference from the stent-assisted coiling group(10.7%;P>0.999).Conclusion For unruptured intracranialposterior circulation aneurysms,both flow diverters and stent-assisted coiling demonstrate good clinical outcomes and comparable aneurysm occlusion rates,with low perioperative complication rates.
