Efficacy of flow diverter devices in the treatment of intracranial vertebral artery dissecting aneurysms

Abstract:Objective To evaluate the efficacy and safety of flow diverter(FD)devices in treating intracranial vertebral artery dissecting aneurysms(IVADAs).Methods The clinical data of 41 IVADA patients treated with FD between July 2018 and June 2024 were retrospectively analyzed.Results Among the 41 cases,14 involved ruptured aneurysms and 27 unruptured aneurysms.All procedures were successfully performed,with 41 FDs implanted.Coil embolization was adjunctively used in 21 cases.No intraoperative aneurysm rupture or hemorrhage occurred.Immediate post-procedural Raymond-Roy occlusion classification showed grade Ⅰ in 10 cases,grade Ⅱ in 7,and grade Ⅲ in 24.Postoperative ischemic complications occurred in 7 patients,and 1 case had its treatment withdrawn after remaining in a persistent coma for two months.No hemorrhagic complications or procedure-related mortality occurred.Forty patients were followed for 7~24 months(mean,13±6 months)after discharge.Modified Rankin Scale scores at 6 months postoperatively were 0 in 33 cases,1 in 4,2 in 2,and 4 in 1.Follow-up DSA within 6~12 months revealed OKM grade D in 36 cases and grade C in 5.Conclusion FD treatment for IVADAs is relatively straightforward,with few complications,high aneurysm healing rates,and effective vascular reconstruction.However,ischemic complications in ruptured IVADAs warrant particular attention.
