Pipeline embolization device combined with coil embolization for ruptured vertebral artery dissecting aneurysms:a case report

Abstract:Vertebral artery dissecting aneurysms(VADAs)are clinically rare but carry high risks of rupture,disability,and mortality.Once ruptured,they require aggressive surgical intervention.Microsurgical treatment of aneurysms at this location is challenging,while standalone stent-assisted coiling has high recurrence rates.This article reports a case of a 40-year-old male who was transferred from the orthopedics department of Binzhou People's Hospital to the neurosurgery department due to sudden headache and syncope for 1 hour one day after open reduction and internal fixation of a right radial bone fracture.Emergency cranial CT revealed subarachnoid hemorrhage(predominantly anterolateral to the pons and medulla oblongata).Head-neck CTA and cerebral angiography confirmed a right VADA.Treatment involved Pipeline Embolization Device(PED)placement combined with coil embolization.Immediate post-procedural angiography showed complete occlusion at the rupture site,with patency of the right vertebral artery,right posterior inferior cerebellar artery,and right anterior inferior cerebellar artery,along with optimal stent apposition.Six-month follow-up angiography demonstrated no recurrence.Although PED for ruptured VADAs remains controversial,this combined approach represents an effective and viable therapeutic option for select cases to prevent re-rupture.
