Safety and feasibility of non-suturing the dura mater in decompressive craniectomy for traumatic brain injury

Abstract:Objective To evaluate the safety and feasibility of leaving the dura mater open during decompressive craniectomy in patients with traumatic brain injury(TBI).Methods A retrospective analysis was conducted on clinical data from 86 TBI patients who underwent decompressive craniectomy between March 2021 and March 2024.Results Among the patients,31 received coverage of dural defects with an artificial dural without suturing(non-sutured group),while 55 underwent dural repair with suturing artificial dural patch(sutured group).The non-sutured group exhibited significantly shorter operative time,reduced intraoperative blood loss,and shorter hospital stay compared to the sutured group(P<0.05).No statistically significant differences were observed between the two groups in 3-month Glasgow Outcome Scale(GOS)scores or perioperative complication rates(P>0.05).Conclusion For appropriately selected TBI patients,omitting dural suturing during decompressive craniectomy is a safe and feasible technique,offering advantages such as reduced operative time,minimized surgical trauma,and shorter hospitalization.
