Unilateral supraorbital endoscopic evacuation of bilateral frontal contusions and hematoma:a case report

Abstract:Bilateral frontal lobe contusions with hematoma formation,a common subtype of traumatic brain injury,carries a high risk of central brain herniation and mortality.Traditional surgical approaches,such as bicoronal incisions with bifrontal craniotomy,are associated with prolonged operative time,significant trauma,substantial intraoperative blood loss,delayed recovery,and frequent complications.While endoscopic surgery via a supraorbital approach has been reported for unilateral frontal contusions and hematomas,its application in bilateral cases remains unexplored.We present a 57-year-old male with bilateral frontal and right temporal contusions(total hematoma volume:49.7 ml on CT).Endoscopic hematoma evacuation was performed via a left unilateral supraorbital approach,achieving bilateral frontal hematoma clearance.Postoperative 3D Slicer reconstruction confirmed a residual hematoma volume of 1.0 ml(98%clearance rate).The patient regained consciousness on postoperative day 1,was discharged on day 16,and exhibited no neurological deficits at 10-month follow-up.This case demonstrates that unilateral supraorbital endoscopic surgery for bilateral frontal contusions offers advantages including reduced operative time,minimal blood loss,rapid neurological recovery,and shortened hospitalization,providing a novel minimally invasive strategy for complex bilateral injuries.
