Association between DSC-PWI imaging characteristics of glioblastoma and IDH1mutation status

Abstract:Objective To investigate the association between dynamic susceptibility contrast perfusion-weighted imaging(DSC-PWI)characteristics of glioblastoma(GBM)and isocitrate dehydrogenase 1(IDH1)mutation status.Methods A retrospective analysis was conducted on clinical data from 51 GBM patients treated between January 2019 and January 2023.All patients underwent preoperative DSC-PWI,and mean relative cerebral blood volume(rCBVmean)and maximum relative cerebral blood volume(rCBVmax)were calculated.Tumor IDH1 mutation status was assessed using immunohistochemistry.Results Among the 51 patients,11 were IDH1-mutant and 40 were IDH1-wildtype.The rCBVmean in IDH1-mutant GBM was significantly lower than that in IDH1-wildtype GBM(P<0.05),while no statistical difference was observed in rCBVmax(P>0.05).The area under the receiver operating characteristic(ROC)curve for rCBVmean in predicting IDH1 mutation was 0.781(95%CI 0.643~0.884).A cutoff value of rCBVmean≤4.24 yielded a sensitivity of 90.91%and specificity of 72.50%for predicting IDH1 mutation.Conclusion DSC-PWI-derived rCBVmean may serve as a non-invasive predictor of IDH1 mutation status in GBM patients,with lower rCBVmean values indicating a higher likelihood of IDH1 mutation.
