3D Slicer-based three-dimensional reconstruction technology-assisted single burr hole microvascular decompression for primary trigeminal neuralgia and hemifacial spasm

Abstract:Objective To evaluate the efficacy of 3D Slicer-based three-dimensional(3D)reconstruction technology-assisted single burr hole microvascular decompression(MVD)in the treatment of primary trigeminal neuralgia(PTN)and hemifacial spasm(HFS).Methods The clinical data of 32 patients with TN or HFS treated with single burr hole MVD from May 2022 to August 2023 were retrospectively analyzed.Preoperative cranial CT data were imported into 3D Slicer software for 3D reconstruction to map the projections of the transverse sinus and sigmoid sinus onto the skull surface.The optimal burr hole location was determined based on their anatomical relationships with the asterion and digastric groove.Additionally,3.0 T MRI raw data in DICOM format were imported into 3D Slicer:3D-TOF sequences were used for arterial vessel segmentation and reconstruction,while 3D-FIESTA sequences were applied for brainstem and cranial nerve segmentation.Results The mean operative time was(102±30)minutes,with craniotomy and closure completed in(25±6)minutes.The single burr hole provided sufficient exposure of the brainstem entry zones and intracranial courses of the trigeminal and facial-acoustic nerve complexes,enabling effective surgical intervention.Postoperative complications included delayed wound healing due to hemorrhage in 1 case and transient facial numbness in 1 case;no cerebrospinal fluid leakage or infections occurred.All 32 patients were followed for 8~15 months(mean,11.0±3.4 months).Complete symptom relief was achieved in 31 cases(96.9%),with partial improvement in 1 case(3.1%),yielding an overall effectiveness rate of 100%.No recurrence or symptom aggravation was observed.Three-month postoperative cranial CT with 3D reconstruction confirmed satisfactory burr hole healing.Conclusion The 3D Slicer-assisted single burr hole MVD technique offers advantages including simplified craniotomy and closure,minimal surgical trauma,adequate operative field exposure,low complication rates,and rapid recovery.It demonstrates favorable efficacy in treating PTN and HFS.
