Surgical treatment of Shamblin Ⅱ carotid body tumor under TCD monitoring:a case report

Abstract:Carotid body tumor(CBT),as a rare neuroendocrine neoplasm,poses significant surgical challenges due to its intricate anatomical relationship with the carotid sheath and cranial nerves.This study presents a representative case:a 43-year-old male patient was admitted with a 1-year history of progressive enlargement of a pulsatile,painless mass in the left neck.Imaging evaluations revealed the following:Color Doppler ultrasound identified a 4.5×4.0×3.0 cm well-defined hypoechoic mass at the left carotid bifurcation with rich internal vascularity;Three-dimensional CT angiography demonstrated characteristic splaying of the internal and external carotid arteries,showing a lobulated heterogeneously enhanced lesion;MRI exhibited isointense signal to muscle,and clear demarcation from the vascular sheath on contrast-enhanced sequences.Following comprehensive preoperative evaluation,complete tumor resection was successfully performed under real-time transcranial Doppler(TCD)cerebral blood flow monitoring,with preserved carotid integrity.Histopathological examination confirmed the diagnosis of CBT.No recurrence or neurological deficits were observed during the 1-year follow-up.This case highlights that integrating precise imaging assessment with TCD dynamic monitoring technology significantly reduces perioperative cerebrovascular complications and nerve injury risks,achieving the dual objectives of total tumor resection and neurological preservation in CBT surgery.
