Abstract:Ewing sarcoma is a highly malignant tumor that typically affects the skeletal system. It is rare in extramedullary soft tissues, and primary Ewing sarcoma in the intracranial region is even more uncommon. This paper reports a case of primary intracerebral Ewing sarcoma originated from the pia mater in a 16-year-old young woman.Plain brain MRI scan revealed an irregular cystic and solid mass in the right temporal lobe and basal ganglia.Intraoperative rapid frozen pathological examination indicated small round cell tumors. The postoperative pathological diagnosis was primary intracranial Ewing sarcoma. One month after the operation, vincristine, doxorubicin, cyclophosphamide, ifosfamide, and etoposide were administered alternately for chemotherapy, along with local radiotherapy to the brain. After three cycles of chemotherapy, the patient was in good health condition.A review brain CT scan showed no residual tumor tissue;no tumor cells were detected in the cerebrospinal fluid cytological examination. In summary, primary intracranial Ewing sarcoma is clinically rare, with non-specific imaging manifestations and a high possibility of misdiagnosis. Therefore,in clinical practice, when encountering similar cases,Ewing sarcoma should be included in the range of differential diagnoses.Meanwhile,an early and clear diagnosis is a prerequisite for improving the prognosis of patients.
