Efficacy of neuroendoscopic surgery for patients with traumatic intracerebral hematoma

Abstract:Objective To investigate the clinical effectiveness of neuroendoscopic surgery for patients with traumatic intracerebral hematoma.Methods The clinical data of 24 patients with traumatic intracerebral hematoma who underwent neuroendoscopic surgery from March 2019 to June 2023 were retrospectively analyzed.Results The hematoma was mainly located in the frontal and temporal lobes,with a volume ranging from 25.6 to 68.8 ml,averaging(38.6±12.5)ml.The operation time ranged from 72 minutes to 149 minutes,averaging(113.7±21.7)minutes.The reexamination of head CT within 24 hours after the operation revealed that the residual hematoma volume ranged from 0.8 ml to 13.2 ml,averaging(4.3±2.7)ml;the hematoma clearance rate ranged from 76.1%to 98.4%,averaging(88.4±4.4)%.Postoperative complications included rebleeding in the surgical area in 1 case,subcutaneous effusion in 1 case,and epilepsy in 2 cases,stress-induced ulcer in 2 cases,and pulmonary infection in 4 cases,all of which improved through conservative treatment.During the 3-month follow-up after the operation,15 patients had a Glasgow Outcome Scale score of 5,8 had a score of 4,and 1 had a score of 3;the good prognosis rate was 95.8%.Conclusion Neuroendoscopic surgery for traumatic intracerebral hematoma has the advantages of less surgical trauma,satisfactory hematoma clearance,fewer postoperative complications,and favorable surgical outcomes.
