A case of solitary fibrous tumor with multiple recurrences and malignant transformation adjacent to the sagittal sinus

Abstract:Solitary fibrous tumor(SFT)is a rare mesenchymal neoplasm characterized by fibroblast-like spindle cell proliferation,predominantly exhibiting benign behavior,with 10%~20%demonstrating malignant potential.This report presents a rare case of recurrent intracranial SFT adjacent to the sagittal sinus with malignant transformation in a 67-year-old male.The patient underwent initial tumor resection 4 years prior(pathological diagnosis unavailable).Two years later,MRI-confirmed recurrence prompted a second surgery,revealing a benign SFT(positive for STAT6 immunohistochemistry).After an additional 2-year interval,a third recurrence with right parietal lobe invasion was surgically excised,and histopathology confirmed progression to malignant SFT(mitotic count>4/10 HPF,necrosis,and nuclear atypia).Postoperative management included adjuvant radiotherapy(total dose:54 Gy)and temozolomide chemotherapy.Follow-up CT at 9 months demonstrated no recurrence.Key implications include:Intracranial SFT carries risks of recurrence and malignant evolution,necessitating gross total resection;multimodal therapy(radiotherapy and alkylating agents)is critical for recurrent or malignant cases;long-term surveillance should prioritize parasagittal and meningeal regions.
