Efficacy of transorbital endoscopic neurosurgery for patients with sphenoid ridge meningiomas

Abstract:Objective To evaluate the clinical efficacy of transorbital endoscopic neurosurgery for patients with sphenoid ridge meningiomas.Methods A retrospective analysis was conducted on the clinical data of 17 patients with sphenoid ridge meningiomas who underwent transorbital endoscopic neurosurgery between September 2016 and December 2021.Results Seven patients(41.2%)achieved Simpson gradeⅠ/Ⅱresection,and the other 10 patients achieved Simpson gradeⅢresection.One week postoperatively,the Karnofsky Performance Scale(KPS)score[(86.6±7.0)]was significantly higher than preoperatively[(79.3±8.4),P=0.015].At three months postoperatively,the KPS score further improved to[(94.0±7.1),P<0.001].The modified Rankin Scale(mRS)score one week postoperatively[(1.2±1.0)]was significantly lower than preoperatively[(2.4±1.2),P=0.035].Postoperative complications included cerebrospinal fluid leakage in one case,ptosis in one case,and periorbital edema in three cases.The median follow-up duration was 15.9 months(range:6~43 months).No tumor recurrence or progression was observed during the follow-up period.Conclusion The transorbital approach provides direct access to the sphenoid bone,and transorbital endoscopic neurosurgery is a safe and effective method for resecting sphenoid ridge meningiomas.However,the indications for this approach should be carefully evaluated.
