Risk factors for ventilator-associated pneumonia caused by Staphylococcus aureus in patients with severe traumatic brain injury

Abstract:Objective To explore the risk factors of ventilator-associated pneumonia(VAP)caused by Staphylococcus aureus in patients with severe traumatic brain injury(sTBI).Methods A retrospective analysis was conducted on the clinical data of 140 sTBI patients who developed VAP in our hospital from March 2021 to February 2023.Multivariate logistic regression model was used to analyze the influencing factors of VAP cuased by Staphylococcus aureus,and ROC curve was used to analyze the predictive efficacy.Results According to the pathogen detection results,39 cases were VAP VAP cuased by Staphylococcus aureus,and 101 cases were VAP caused by other factors.A total of 154 pathogenic bacteria were detected in 140 cases of VAP,among which 8 cases were mixed bacteria;the top three were Staphylococcus aureus,Klebsiella pneumoniae,and Acinetobacter baumannii.Multivariate logistic regression analysis showed that age≥60 years,smoking history,long mechanical ventilation time,tracheotomy,and serum albumin<30 g/L were independent risk factors for VAP cuased by Staphylococcus aureus in sTBI patients.ROC curve analysis showed that the area under the curve of this logistic regression model was 0.897(95%CI 0.838~0.957),the sensitivity was 0.795,and the specificity was 0.861.Conclusion Advanced age,smoking history,mechanical ventilation time,tracheotomy,and serum albumin level are closely related to VAP caused by Staphylococcus aureus in sTBI patients,thus corresponding measures can be taken in clinical practice for prevention and treatment.
