Effect of surgical timing on the efficacy of cranial puncture and soft channel catheter drainage for patients with traumatic brain injury complicated with intracerebral hematoma

Abstract:Objective To investigate the influence of surgical timing on the efficacy of cranial puncture and soft channel catheter drainage for patients with traumatic brain injury(TBI)complicated with intracerebral hematoma.Methods A retrospective analysis was performed on the clinical data of 65 patients with moderate TBI complicated with intracerebral hematoma who underwent cranial puncture and soft channel catheter drainage from March 2019 to March 2024.The patients were divided into the early group(within 12 hours after injury,n=31)and the conventional group(12~48 hours after injury,n=34)based on the surgical timing.The Glasgow Coma Scale(GCS)was utilized to assess the patients'consciousness status 7 days after the surgery,and the Glasgow Outcome Scale(GOS)was employed to evaluate the prognosis 6 months after the surgery,with a score of 4~5 indicating a good prognosis.The Self-Rating Anxiety and Depression Scales were used to assess the levels of anxiety and depression.Results Seven days after the surgery,the GCS score of the early group(13.4±1.9)was significantly higher than that(12.2±2.4)of the conventional group(P=0.027),and the volume of perihematomal edema of the early group[(19.26±11.46)cm3]was significantly smaller than that[(27.28±13.20)cm3]of the conventional group(P=0.012).At 6 months post-operation,there was no statistically significant difference in the good prognosis rate between the early group(83.9%)and the conventional group(70.6%;P=0.204),but the rate of anxiety in the early group(19.4%)was significantly lower than that(44.1%)in the conventional group(P=0.033).Conclusions For patients with moderate TBI accompanied by intracerebral hematoma,the early implementation of cranial puncture and soft channel catheter drainage can alleviate cerebral edema more effectively,improve the patients'consciousness status,and contribute to reducing the long-term incidence of anxiety in patients.
