Clinical features and surgical outcomes of posterior cortex epilepsy

Abstract:Objective To investigate the clinical features and surgical outcomes of posterior cortex epilepsy(PCEs).Methods The clinical data of 71 patients with PCEs who underwent resection from January 2006 to January 2020 were retrospectively analyzed.Results The seizure focus was limited to one cerebral lobe in 34 patients(single lobe group),and involved multiple cerebral lobes in 37 patients(multi-lobe group).The postoperative follow-up ranged from 2 to 8 years,with an average of(4.1±1.3)years.The overall seizure control rate was 63.4%after surgery.The seizure control rate of occipital lobe epilepsy was 94.1%,that of parietal lobe epilepsy was 82.2%,that of occipital lobe+posterior temporal lobe epilepsy was 50.0%,that of occipital lobe+parietal lobe epilepsy was 46.7%,that of parietal lobe+posterior temporal lobe epilepsy was 33.3%,and that of occipital lobe+parietal lobe+posterior temporal lobe epilepsy was 0%.The seizure control rate in the single lobe group(88.2%)was significantly higher than that(40.5%)in the multi-lobe group(P<0.001).There were statistical differences in gender composition,risk factors for epilepsy,disease course,frequency of seizure,types of seizure,and pathological characteristics between the single lobe group and the multi-lobe group(P<0.05).The multivariate logistic regression analysis showed that disease duration>5 years(OR=2.055;95%CI 1.301~3.247;P=0.015),multi-lobe involvement(OR=2.538;95%CI 1.627~3.959;P=0.006),and partial resection of the epileptogenic focus(OR=1.537;95%CI 1.092~2.268;P=0.011)were independent risk factors for poor seizure control after surgery.Conclusions For PCEs,complete resection of the epileptogenic focus can achieve good control effects,with better outcomes in patients with unilateral cerebral lobe involvement;however,patients with a disease history more than 5 years,multiple cerebral lobes involvement,and partial resection of the epileptogenic focus are more likely to experience seizures after surgery.
