Progress of surgical treatment for refractory schizophrenia

Abstract:Schizophrenia is a chronic and severely debilitating psychiatric disorder characterized by disturbances in thought,emotion,behavior,and cognitive function. The prevalence of schizophrenia in China is approximately 1%,with estimates indicating that 20%~30% of patients may eventually develop refractory schizophrenia. Refractory schizophrenia exhibits suboptimal responsiveness to conventional antipsychotic medications,posing significant clinical challenges. Advances in neurosurgical techniques have led to the consideration of surgical interventions for refractory schizophrenia,including stereotactic focal lesioning and deep brain stimulation (DBS). As our understanding of cerebral neural circuits deepens and modern stereotactic and neuroimaging technologies advance,neurosurgical approaches for refractory schizophrenia have garnered increased attention. These methods now enable more minimally invasive and precise target lesioning. Particularly,the development of DBS has significantly enhanced the potential of neurosurgery in treating mental disorders. This review aims to summarize recent advancements in neurocircuit imaging research and neurosurgical treatments for refractory schizophrenia,providing insights for comprehensive management strategies for refractory schizophrenia.
