Application of magnetic resonance diffusion imaging in the surgical management for drug-resistant epilepsy

Abstract:Epilepsy is a disorder of the central nervous system resulting from abnormal electrical discharges of local neurons and their propagation through neural networks. Approximately one-third of patients with epilepsy fail to control seizures despite taking two or more antiepileptic drugs,which is known as drug-resistant epilepsy (DRE). Surgical resection of the epileptogenic focus is an effective approach for treating DRE. However,due to the complex pathogenesis and diverse clinical manifestations of DRE,surgical treatment encounters numerous challenges,such as inaccurate preoperative etiological diagnosis,potential damage to functional areas during surgery,and suboptimal postoperative prognosis assessment. In recent years,magnetic resonance diffusion imaging has been constantly evolving and possesses unique advantages in analyzing white matter structure alterations and brain structural networks. It plays an increasingly crucial role in preoperative etiological diagnosis,intraoperative protection of functional areas,and postoperative prognosis evaluation for DRE surgery. This article reviews the application advancements of magnetic resonance diffusion imaging in the surgical treatment of DRE,with the expectation of providing clinical assistance.
