Effects of hospital-based transitional care on rehabilitation of patients with stroke:a meta-analysis

Abstract:Objective To evaluate the effects of hospital-based transitional care intervention on rehabilitation of patients with stroke. Method The databases including Cochrane Library, PubMed, Embase, CNKI, Wanfang Database and VIP were systematically searched for the original articles about the effects of hospital-based transitional care on the rehabilitation of the patients with stroke published from January,2000 to August,2017.Meta analysis was performed by RevMan 5.2.Results Fourteen articles which met the inclusion criteria were enrolled in the meta-analysis and included 1 331 patients,of whom,667 were in the experimental group and 664 in the control group. Meta-analysis showed that the modified Barthel indext (MD=15.87; 95% CI: 11.27~20.46; P<0.001), stroke specialized quality of life scale(MD=14.76;95% CI:3.2~26.43;P=0.01)and Fugl-Meyer assessment scores(MD=15.78;95% CI:8.63~22.93;P<0.001)were significantly higher and the rehospitalization rate(OR=0.19;95% CI:0.05~0.77;P=0.02)was significantly lower in the experimental group than those in the control groups.Conclusion The hospital-based transitional care intervention is very helpful to improving the quality of life and promoting the rehabilitation in patients with stroke.
