Analysis of factors related to coagulopathy in patients with traumatic brain injury

Abstract:Objective To explore the factors related to coagulopathy in the patients with traumatic brain injury(TBI).Methods Of 136 patients with TBI treated in our hospital from January,2012 to December,2015,51 suffered from coagulopathy(coagulopathy group) and 85 not(non-coagulopathy group).Multiple factors logistic regression analysis was performed to screen out the independent factors related to coagulopathy. Results The monofactorial analysis showed that patients with abnormal levels of blood fibrinogen (Fib) accounted for 29.4% of all the patients with TBI and patients with abnormal international normalized ratio(INR)accounted for 19.1% of all the patients with TBI.There were significant differences in GCS scores,blood glucose levels,midline shift on CT,skull fracture,acute subdural hematoma and intraventricular bleeding between two groups(P<0.05). the multiple factors logistic regression analysis showed that GCS scores≤8 point,blood glucose level≥20.1 mmol/L,midline shift on CT image,acute subdural hematoma and intraventricular bleeding were the independent risk factors of coagulopathy(P<0.05).Conclusions The main forms of coagulopathy in the patients with TBI were abnormal FIB and INR.The independent risk factors of coagulopathy include GCS scores≤8 point,blood glucose level≥20.1 mmol/L,midline shift on CT image,acute subdural hematoma and intraventricular bleeding in the patients with TBI.
