Risk factors for prognosis of female prolactinoma patients after transphenoidal surgery

Abstract:Objective To explore the independent risk factors for the prognosis of female prolactinoma patients treated by transsphenoidal surgery.Methods The clinical data of 138 female prolactinoma patients,who firstly underwent surgery from 2011 to 2014,were analyzed retrospectively.Results The initial remission rate was 80.4% in 138 patients.The remission rate was 80.7% 3 months after surgery.Of the patient with preoperative prolactin <500 ng/ml,the initial remission rate was 95.3%,and 3-month postoperative remission rate was 87.6%.The initial remission rate of micro-adenoma without cavernous sinus invasion was 93.9%,and the remission rate was 91.3% 3 moths after suregry.Multivariate Logistic regression analysis showed that tumor invasion of cavernous sinus and preoperative hyperprolactinemia were independent risk factors for the prognosis (P<0.05).Of 114 patients followed up for 3 to 51 months (mean,20.1±12.3 months),the recurrence rate was 5.4%.Conclusion The transphenoidal surgery is a good choice for female prolactinoma patients without cavernous sinus invasion or with preoperative prolactin less than 500 ng/ml.
