Relationship of critical illness-related corticosteroid insufficiency with prognoses in patients with severe craniocerebral injury

Abstract:Objective To observe the incidence of critical illness-related corticosteroid insufficiency (CIRCI) and the relationship between CIRCI and the prognoses in patients with severe craniocerebral injury. Methods The plasma level of the corticosteroid was determined and ACTH-stimulation test was performed in 49 patients with severe craniocerebral injury, and then they were divided into two groups, i.e. CIRCI and no CIRCI groups according to diagnostic standard of CIRCI. The APACHE-Ⅱ score and the 28-day mortality were compared between both the groups. Results The incidence of CIRCI in patients with severe craniocerebral injury was 67.3%(33/49). The APACHE-Ⅱscore [(23.56±5.74) points] was significantly higher in CIRCI group than that [(19.50±6.77) points] in no CIRICI group (P<0.05). The 28-day mortality (63.6%, 21/33) was significantly higher in CIRCI group than that (31.25%, 5/16) in no CIRCI group (P<0.05). Conclusion The determination of CIRCI is very helpful to the assessment of the condition severity and the prognoses in patients with severe craniocerebral injury.
