Research progress on ECPR combined with TTM for improving neurological outcomes after cardiac arrest

Abstract:Survivors of cardiac arrest(CA)often sustain severe neurological impairments due to global cerebral ischemia-reperfusion injury,which is a major determinant of long-term quality of life.Extracorporeal cardiopulmonary resuscitation(ECPR)can provide superior circulatory support compared to conventional cardiopulmonary resuscitation(CCPR)for the patients with refractory CA.Targeted temperature management(TTM)is a core neuroprotective strategy for improving neurological outcomes after CA.However,the efficacy and optimal protocol for applying TTM in the patients supported by ECPR remain subjects of significant debate.This review systematically elaborates the neuroprotective mechanisms and clinical evidence regarding the combined application of ECPR and TTM.It provides an in-depth analysis of key factors influencing its efficacy,such as treatment time windows and target temperature,and highlights the limitations of current research,including the lack of prospective randomized controlled trials and high heterogeneity.Future research directions,including personalized TTM protocols and the integration of multimodal neuromonitoring and artificial intelligence technologies,are also discussed in order to provide assistance in improving neurological outcomes for CA patients.
