Application and development of prolonged prone position ventilation in acute respiratory distress syndrome

Abstract:Acute respiratory distress syndrome(ARDS),as a high mortality disease,continues to evolve in its treatment strategies.Among these,prone position ventilation(PPV),which optimizes the ventilation-perfusion ratio in the lungs and promotes alveolar recruitment through gravitational effects,has been demonstrated to significantly reduce patient mortality.Based on this,the prolonged prone position ventilation(PPPV)strategy enables more sustained improvement in oxygenation and reduces ventilator-induced lung injury by prolonging the duration of each session.Additionally,it helps mitigate the risks such as catheter displacement and pressure injuries by decreasing the frequency of positional changes.This study systematically reviews the pathological mechanisms and key therapeutic measures of ARDS,the developmental history of PPV,and provides an in-depth exploration of the clinical application and optimization strategies of PPPV.It also analyzes potential complications associated with PPPV and proposes evidence-based comprehensive prevention and management plans to advance PPPV toward safer and more effective implementation,ultimately improving patient clinical outcomes.
