Adipose-derived mesenchymal stem cells for the treatment of sepsis:A systematic review and Meta analysis based on animal studies

Abstract:Objective To evaluate the efficacy and safety of adipose-derived mesenchymal stem cells(AD-MSCs)in the treatment of sepsis.Methods A comprehensive search of PubMed,Embase,Web of Science,Scopus,CNKI,Wanfang,and SinoMed was conducted to collect animal experiments on AD-MSCs for the treatment of sepsis.The search covered English databases from inception to December 18,2024,and Chinese databases from inception to April 8,2025.Results This study included 17 studies.The Meta-analysis of mortality showed that AD-MSCs reduced the risk of death in septic animals at 24 h(RR=0.59,P<0.000 1),48 h(RR=0.66,P=0.002 0),and72 h(RR=0.62,P<0.000 1).Further analyses indicated that administration within 1 hour was more effective than administration after>1 hour.Moreover,AD-MSCs reduced mortality in both CLP and nonCLP models,with a more pronounced benefit observed in nonCLP models.No significant differences in efficacy were found among different cell doses or animal species.In addition,AD-MSCs significantly decreased inflammatory cytokine levels,including serum TNF-α at 6 h and 72 h and serum IL-6 at 6 h,and reduced AST and ALT levels,while the effect on BUN was not statistically significant.Conclusions Current animal evidence suggests that AD-MSCs can effectively inhibit inflammatory response,reduce ALT,AST and BUN levels,and significantly reduce short-term mortality in animal models of sepsis.
