Cell-free DNA in sepsis:from generation mechanisms to clinical implementation

Abstract:Sepsis is a severe systemic inflammatory response syndrome(SIRS)triggered by infection.Early identification and accurate prognostic assessment are crucial for improving patient outcomes.Cell-free DNA(cfDNA),as an emerging biomarker,has garnered significant research attention in the field of sepsis in recent years.Studies indicate its involvement in the pathogenesis and progression of the disease and suggest its potential value in prognostic assessment.This article systematically reviews the generation mechanisms of cfDNA(including release pathways mediated by apoptosis,necrosis,and immune responses),its clinical applications(such as early identification,severity assessment,and mortality prediction),and its complementary role with conventional scoring systems and biomarkers.By analyzing the relationship of cfDNA levels with the degree of organ failure,treatment response,and long-term prognosis in septic patients,this review aims to provide valuable insights for clinical practice.
