A meta-analysis of the efficacy and safety of intra-aortic balloon pump combined with percutaneous coronary intervention in the treatment of acute myocardial infarction

Abstract:Objective To assess the efficacy and safety of intra-aortic balloon pump(IABP)combined with percutaneous coronary intervention(PCI)in the patients with acute myocardial infarction(AMI).Methods A systematic review was conducted on randomized controlled trials(RCTs)published in databases including PubMed,Embase,Wanfang and CNKI.Studies included AMI patients treated with IABP combined with or without PCI,and the data were analyzed by using RevMan 5.4 software.Results The results indicated that the combination therapy of IABP with PCI reduced the risk of mortality(RR=0.73,95%CI0.61-0.86,P=0.0002).Subgroup analysis showed greater benefits in the patients receiving timely IABP during PCI and those without concomitant cardiogenic shock(CS).Long-term follow-up data demonstrated IABP improved the prognosis(RR=0.84,95%CI 0.75-0.95,P=0.000 4).IABP in combination with PCI effectively reduced the risk of recurrent infarction,malignant arrhythmias,and heart failure exacerbations,but increased the risk of bleeding.Conclusions Combination therapy of IABP and PCI significantly reduces mortality risk and improves cardiac function in the patients with AMI,particularly benefiting AMI patients without concurrent CS.However,IABP treatment carries a risk of bleeding,necessitating careful consideration of risk-benefit profiles in clinical practice.
