The predictive value of prognostic nutritional index and heparin-binding protein for 28-day mortality risk in the patients with acute mesenteric ischemia

Abstract:Objective To investigate the predictive value of the prognostic nutritional index(PNI)and heparin-binding protein(HBP)for 28-day mortality risk in the patients with acute mesenteric ischemia(AMI).Methods A retrospective analysis was conducted on 99 AMI patients admitted to the Emergency Surgery Department of the First Affiliated Hospital of Bengbu Medical University from January 2019 to December 2023.Based on prognosis,the patients were divided into a survival group(n=59)and a death group(n=40).General data and laboratory results within 24 h of admission were collected,and the indicators such as PNI and acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)scores were calculated.Logistic regression analysis was used to identify the factors affecting the prognosis,while receiver operating characteristic(ROC)curves and Kaplan-Meier survival curves were plotted to evaluate the predictive efficacy of PNI and HBP for 28-day mortality in AMI patients.Results The death group had significantly older age,higher APACHE Ⅱ scores,HBP levels and lactate values compared to the survival group(all P<0.01),while PNI scores were significantly lower in the death group(P<0.01).HBP was positively correlated with APACHE Ⅱ scores(P<0.05),whereas PNI was negatively correlated with APACHE Ⅱ scores(P<0.05).Logistic regression analysis identified age,APACHE Ⅱ scores,HBP and lactate were risk factors for the prognosis,but PNI was a protective factor(all P<0.05).ROC analysis revealed area under the curve(AUC)values were 0.672,0.713,0.696 and 0.790 for age,APACHE Ⅱ,HBP and lactate,respectively(all P<0.01),with the sensitivities of 0.475,0.775,0.500,0.750 and the specificities of 0.831,0.610,0.814,0.864.The AUC for PNI was 0.755(P<0.01),with a sensitivity of 0.675 and specificity of 0.797.Kaplan-Meier survival curves demonstrated significantly lower 28-day cumulative survival rates in AMI patients with HBP>65.345 ng/mL and PNI≤43.50 scores(both P<0.01).Conclusions PNI and HBP have predictive value for 28-day mortality risk in AMI patients.The lower PNI scores and elevated HBP levels will increase 28-day mortality risk in AMI patients.
