Correlation of triglyceride-glucose index and systemic immune-inflammation index with acute ischemic stroke

Abstract:Objective To explore the correlation of the triglyceride-glucose(TyG)index and systemic immune-inflammation index(SII)with acute ischemic stroke(AIS).Methods A total of 287 AIS patients admitted to Zhujiang Hospital of Southern Medical University from January 2022 to December 2023 were selected as the AIS group,and non-AIS patients(non-ischemic encephalopathy)in the same period were selected as the control group.The general data,biochemical indexes and laboratory indexes of the two groups were compared,and the correlation of TyG and SII with AIS was further analyzed by Spearman rank correlation and Logistic regression analysis.Receiver operating characteristic(ROC)curve analysis was used to analyze the diagnostic efficacy of SII and TyG for AIS.Results The national institute of health stroke scale(NIHSS)score on admission[score:6.00(3.00,9.00)vs.2.00(1.00,4.00)],triglyceride[TG,mmol/L:2.74(1.75,3.12)vs.1.28(1.16,1.49)],fasting blood glucose[FBG,mmol/L:9.37(7.17,11.05)vs.5.81(4.30,7.58)],TyG[8.14(7.68,8.48)vs.7.13(6.79,7.53)],platelet(PLT,x 109/L:199.64+51.36 vs.176.14±30.98),neutrophil[NEU,x109/L:6.54(5.30,8.21)vs.4.20(2.99,5.42)]andSII[798.56(538.84,1219.20)vs.299.06(170.22,467.65)]were all higher in the AIS group than in the control group(P<0.05),while lymphocyte[LYM,x109/L:1.59(1.00,2.20)vs.2.53(1.69,3.27),P<0.05]of the AIS group was lower than that of control group.Logistic regression analysis further confirmed that NIHSS score on admission,TyG and SII were independent risk factors for AIS[odds ratio(OR)was 1.350,8.761 and 1.001,95%confidence interval(95%CI)was 1.250-1.458,5.823-13.183 and 1.001-1.002,respectively,P<0.05].ROC curve was used to evaluate the predictive value of NIHSS score on admission,TyG and SII for AIS.When the three indexes were used separately for the diagnosis of AIS,the AUC was 0.763,0.842 and 0.841,respectively.However,the AUC of TyG in combination with SII was significantly increased to 0.891,and the sensitivity and specificity were correspondingly increased(0.774 and 0.839,respectively).Conclusions This study shows that NIHSS score,TyG and SII are independent risk factors for AIS,and the combined use of TyG and SII can significantly enhance the diagnostic efficiency of AIS.
