The predictive value of soluble epoxide hydrolase in the prognosis of patients with hemorrhagic shock

Abstract:Objective To clarify the expression and significance of soluble epoxide hydrolase(sEH)in the patients with hemorrhagic shock.Methods Eighty-four patients with hemorrhagic shock admitted to the Emergency Department of the 908th Hospital of the Joint Logistics Support Force,PLA from January 2023 to September 2024 were selected as the research subjects.Serum samples were collected and various clinical indicators were tested before and after recovery.Serum samples from forty-two healthy volunteers were collected as the control group.The sEH protein content and enzyme activity in serum were measured by using enzyme-linked immunosorbent assay(ELISA).Their prognosis were recorded.Based on multiple organ dysfunction syndrome or death,the patients were divided into poor prognosis group and good prognosis group,the clinical data in two groups were compared.The factors affecting the prognosis of hemorrhagic shock patients were analyzed by univariate and multivariate analysis.Results The level of serum sEH protein and sEH enzyme activity in the shock group before and after recovery were higher than those in the control group[sEH protein(pg/mL):150.81±70.34 and 122.63±50.13 vs.38.72±8.31,sEH enzyme activity(U/L):128.15±64.44 and 108.12±42.77 vs.25.51±7.43,P<0.05].The level of serum sEH protein in the patients with hemorrhagic shock was significantly positively correlated with pulse,and significantly negatively correlated with systolic blood pressure,diastolic blood pressure and hemoglobin.The level of serum sEH protein in hemorrhagic shock patients with poor prognosis was higher than that in hemorrhagic shock patients with good prognosis(pg/mL:197.30±102.06 vs.139.02±54.77).The level of serum sEH protein,systolic blood pressure and hemoglobin were independent factors affecting the prognosis of hemorrhagic shock patients,and the differences were statistically significant(P<0.05).Receiver operating characteristic curve analysis showed that serum sEH protein,systolic blood pressure and hemoglobin had a certain predictive value for the prognosis of hemorrhagic shock patients(P<0.05),the predictive value of the combined detection of three indexes was the highest,the area under curve(AUC)was 0.830.Conclusions The level of serum sEH is highly expressed in the patients with hemorrhagic shock and is closely related to prognosis.The level of serum sEH protein,systolic blood pressure and hemoglobin have high predictive value for the prognosis of hemorrhagic shock and can be used as reference indicators for clinical diagnosis and treatment.
