The predictive value of good outcome following attempted resuscitation score for in-hospital outcome in the patients with cardiac arrest

Abstract:Objective To investigate the predictive value of good outcome following attempted resuscitation(GOFAR)score for restoration of spontaneous circulation(ROSC)in the patients with out-of-hospital cardiac arrest(OHCA).Methods This study was a retrospective study.A total of 114 patients with OHCA admitted to Lishui Central Hospital from April 2022 to March 2024 were selected as the study subjects.According to whether ROSC occurred,they were divided into ROSC group(n=34)and non-ROSC group(n=80).The patient data were collected from the hospital information management system including age,gender,diabetes,hypertension,coronary heart disease,initial monitoring of heart rhythm,no-reflow time,adrenaline dosage,and mild hypothermia treatment.The results of laboratory indexes,including heart rate,body temperature,respiratory rate,mean arterial pressure(MAP)and blood lactic acid(Lac),were recorded for the first time after admission.The GOFAR score of the patients was counted.Results There were significant differences in age,no-reflow time,GOFAR score and Lac between non-ROSC group and ROSC group(P<0.05).Logistic regression analysis showed that age(OR=1.331,95%CI 1.022-1.734),no-reflow time(OR=1.842,95%CI 1.305-2.601),GOFAR score(OR=3.709,95%CI 2.250-6.114)and Lac(OR=2.051,95%CI 1.441-2.919)were the influencing factors of ROSC in OHCA patients(all P<0.05).The area under the receiver operating characteristic(ROC)curve(AUC)of ROSC in OHCA patients predicted by age,no-reflow time,GOFAR score and Lac were 0.839,0.795,0.916 and 0.886,respectively.The AUC value of GOFAR score was the highest,with a sensitivity of 90.58%,a specificity of 84.56%(95%CI 0.866-0.983).Conclusions Age,no-reflow time,GOFAR score and Lac are independent influencing factors of ROSC in OHCA patients.GOFAR score can better predict ROSC and is worthy of clinical promotion.
