Potential association between omentin-1 and prognosis in the patients with sepsis:based on restricted cubic spline and threshold effect analysis

Abstract:Objective To explore the correlation between omentin-1 and inflammatory markers in the patients with sepsis and its effect on the prognosis.Methods A total of 198 patients with sepsis admitted to Shenzhen Longhua District Central Hospital from January 2021 to August 2023 were selected as the research objects.According to the 28-day prognosis,the patients were divided into survival group and death group.The clinical data and omentin-1 levels,C-reactive protein(CRP)and procalcitonin(PCT)were compared between the two groups.Spearman correlation analysis was used to analyze the correlation of omentin-1 with CRP and PCT.Logistic regression was used to analyze the influencing factors of septic prognosis.Restricted cubic spline(RCS)and threshold effect were used to analyze the relationship between omentin-1 and the prognosis of sepsis.Results Among 198 patients with sepsis,145 patients(73.23%)survived and 53 patients(26.77%)died.The levels of CRP,PCT and omentin-1 in the death group were higher than those in the survival group(P<0.05).Spearman correlation analysis showed that omentin-1 was positively correlated with CRP and PCT in the patients with sepsis(r =0.164,0.162,P<0.05).Logistic regression analysis showed that APACHEⅡ score(OR 1.286,95%CI 1.052-1.573),SOFA score(OR 2.731,95%CI 1.530-4.874),CRP(OR 1.053,95%CI 1.012-1.096),PCT(OR 6.187,95%CI 2.553-14.996)and omentin-1(OR 1.007,95%CI 1.002-1.012)were the influencing factors of the prognosis in the patients with sepsis(P<0.05).RCS analysis showed a nonlinear relationship between omentin-1 and the prognosis of sepsis(P<0.001).The threshold effect showed that the breakpoint of omentin-1 on the prognosis of sepsis was 457.81 ng/mL.The probability of death in the patients with omentin-1≥457.81 ng/mL increased with the increase of omentin-1(OR 1.017,95%CI 1.012-1.021,P<0.05).The probability of death in patients with omentin-1<457.81 ng/mL decreased with the increase of omentin-1(OR0.989,95%CI 0.985-0.994,P<0.05).Conclusions Omentin-1 is associated with CRP,PCT and prognosis in the patients with sepsis.The risk of death in the patients with omentin-1≥457.81 ng/mL increases with the increase of omentin-1.
