The effect of rescuer' s position setting on the cardiopulmonary resuscitation quality of chest compression

Abstract:Objective To assess the association between the cardiopulmonary resuscitation(CPR)quality and position settings of standing and kneeling during chest compression.Methods We conducted a prospective,cross-over and randomized trial with clinical students enrolled in the study.The participants received standard training on performing CPR and passed the examination of basic life support,and were divided randomly into two groups.The participants in the two groups separately performed chest compression with standing and kneeling positions in turn for 2 min.The trials were performed with standard manikin models.CPR quality indicator data were recorded by the quality tracking and feedback system automatically.Results 126 participants were included with the average age of 22(with standard deviation of 2),among which 47 patients(37.3%)were male.Records for chest compression with kneeling and standing positions showed statistically significant differences in accuracy[(24.64±25.47)%vs.(31.65±28.16)%,P =0.039],average depth(mm:53.10±10.44 vs.50.50±7.04,P =0.022),and the happening of over-depth compression[(29.51±34.95)%vs.(15.67±24.07)%,P<0.001],without the relation to gender factor.There was no statistically significant difference in compression frequency between two groups.Liner regressions implied that the compression depth during chest compression with standing position was positively correlated with the height of the participants when the bed was fixed(P =0.001,95%CI 0.217-0.857).Conclusions When performing chest compression,the rescuer's position can influence the CPR quality.Standing position is more prone to excessive pressure.Additionally,the depth of compression is positively correlated with the height of the rescuer with a fixed bed height.
