Relationship between international normalized ratio and the mortality in adult septic patients

Abstract:Objective To explore the relationship between the international normalized ratio(INR)and the mortality in adult septic patients based on a large-scale and real-world clinical database.Methods The information about adult septic patients extracted from the Medical Information Mart for Intensive Care-Ⅳ database(MIMIC-Ⅳ)version 2.0 was divided into four groups according to the quartile of the maximum INR within 24 hours after ICU admission.Relationship of different INR levels in four groups with ICU mortality,in-hospital mortality,and 28-day mortality was analyzed.The further association between INR and 28-day mortality in those patients was assessed by using Cox proportional hazards models.The septic patients in the past three years from the Intensive Care Unit of Shaanxi Traditional Chinese Medicine Hospital were extracted for verification.Results INR quartile from the MIMIC-Ⅳ(version 2.0)database was associated with with ICU mortality,in-hospital mortality and 28-day mortality in septic patients(P﹤0.001).The highest INR quartile(INR>2.0)was associated with increased 28-day mortality(HR =1.499,95%CI 1.074-2.091,P = 0.017).Meanwhile,the validation section further stated that INR was associated with septic outcomes.Conclusions INR is an independent risk factor for 28-day mortality in adult septic patients,and large-scale domestic data are needed to define the cut-off value of INR that induces poor outcome.
