Abstract:Objective To explore the clinical effect of vitamin D combined with low dose glucocorticoid in the treatment of children with severe sepsis. Methods 136 children with severe sepsis were randomly divided into group A (conventional treatment,45 cases),group B (routine treatment + low dose glucocorticoid,54 cases) and group C (routine treatment + low dose of glucocorticoid and vitamin D,37 cases),there were no significant difference among general data of the three groups (P > 0.05).Inflammatory infection index,PRISM Ⅲ score,PICU time,28-day mortality and adverse reaction were compared among the three groups. Results The levels of PCT and lactate of the group B and the group C after the treatment of 3 d,7 d and 14 d were significantly lower than those in the group A (P < 0.05),but there was no significant difference among the three groups in CRP (P > 0.05).The PRISM Ⅲ score and PICU time of the group B and the group C were significantly lower than those of group A (P < 0.05).The 28-day mortality of the group B was lower than that of the group A,but there was no significant difference (P > 0.05),and the 28-day mortality of group C was significantly lower than that of group A and B (P < 0.05).Adverse reactions were of no significant difference (P > 0.05) among the three groups. Conclusion The effect of low dose glucocorticoid adjuvant treatment of children with severe sepsis is good;it can improve the control of inflammatory markers and prognosis.Based on this, combined with vitamin D can reduce mortality.
