Effect of regional citrate anticoagulant in continuous blood purification to patients with hepatic dysfunction in ICU

Abstract:Objective To evaluate the safety and efficiency of citrate anticoagulant based contin-uous blood purification in patients with hepatic dysfunction. Methods Forty-six patients with hepatic dysfunction received CVVH based on regional citrate anticoagulant through two way catheter were ob-served. Clotting in filter during CVVH, and the level of APTT, ALT, AST, TBIL, electrolyte, acid-base balance and lactic acid and the incidence of complications, such as severe hypocalcemia, citrate accumu-lation, bleeding were monitored pre and post CVVH. Results Regional citrate anticoagulant showed good effect on CVVH to patients with hepatic dysfunction in ICU with no severe complications. The filters didn't stop due to coagulation during one hundred thirty-eight CVVH, and there was no significance dif-ference in concentrations of APTT, ALT, AST, TBIL, pH, Na+, HCO3-, Ca2+between pre and post treatment (P>0.05). The concentration of K+, lactic acid was significantly lowered post CVVH compared to pre (P<0.05), while it didn't reach clinical significance because the concentration was maintained in normal range. Conclusion Regional citrate based continuous blood purification can be safety and effective used in patients with hepatic dysfunction.
