The influence of goal-directed fluid therapy on perioperative cerebral blood HO-1 and S100-β of gastrointestinal surgery patients

Abstract:Objective To study the influence of goal-directed fluid therapy on perioperative cerebral blood heme oxygenase-1 (HO-1) and S100-β protein of gastrointestinal surgery patients and its possible mechanisms of perioperative cerebral protection.Methods We selected 40 patients with gastrointestinal surgery,which double blind randomly divided into goal-directed liquid treatment group (G group,20 cases) and normal liquid treatment group (N group,20 cases).Respectively extracted venous blood of 5 mL of two groups patients at the time of 10 min before the surgery (T0),1 hour after the started the surgery (T1),30 min before the end of surgery (T2) and 30 min after extubation (T3),measuremented blood osmotic pressure,and while extractedarterial blood and internal jugular vein blood 1 mL did blood gas analysis at the each time,calculated cerebral oxygen uptake rate of two groups,and the extracted internal jugular venous blood 5 mL,detected concentration of heme oxygenase (HO) and S100-β protein in brain blood,and observed the change rule and the differences between two groups.Results Com pared with the N group,the perioperative hemodynamic of G group was more stable,brain tissue oxygen intake which including ERO2,pH,Da-vL was significantly higher and lactate was significantly lower (P < 0.01),the cerebral perfusion (nose temperature)was better(P < 0.05).And compared with the N group,the concentration of HO-1 in cerebral blood of G group increased at the time T1 (P < 0.05),and increased significantly at the time T2 and T3 (P < 0.01),and the concentration of S100-β in cerebral blood significantly reduced at the time T1,T2 and T3(P < 0.01).Conclusion Goal-directed fluid therapy may pronote patients cerebral blood HO-1 increased,protected the brain tissue damage and decreased S100-β generated,which may be related to the method that better maintain perioperative hemodynamic stability and cerebral supply/oxygen supply of gastrointestinal surgery patients,and have significance of perioperative cerebral function protection.
