Use of intravenous immunoglobulin in the emergency department of a general hospital

Abstract:Objective Intravenous immunoglobulin (IVIG) is widely used in various clinical scenarios and is very expensive. Quite a few studies have evaluated current usage of IVIG and the rationale behind it in different countries, resulting in clinical guidelines. There are a lot of patients being treated with IVIG in the emergency department ( ED ) , the audit of this drug in these patients is important. Methods We searched the hospital information system to get all the ED patients who received IVIG during the year of 2013. The medical records were reviewed retrospectively to reveal the indications and quantity was calculated for each indication. Results We confirmed that 162 patients with complete record used IVIG, with a total amount of 11 500 g. Indications most often seen were immune thrombocytopenia purpura, Guillain – Barré syndrome, inflammatory myopathies, and myasthenia gravis. 91. 48% of the indications were acknowledged indications or indications to be assessed. Conclusion In the ED of our hospital, a number of patients received IVIG therapy and most of them have acceptable indications. Further study need to be done from a hospital basis. Trials to evaluate the efficacy in Chinese patients are necessary, so is a domestic guideline of IVIG use.
