The effects of different oxygen concentrations on the respiratory mechanics in mechanical ventilated patients with acute respiratory distress syndrome

Abstract:Objective To investigate the effects of different oxygen concentrations on respiratory mechanics in mechanically ventilated patients with acute respiratory distress syndrome( ARDS) . Methods Eighty patients with ARDS were included in this self - control study. Four different oxygen concentrations ( A 30%, B 40%, C 50%, D 60%) on recruitment maneuver ( RM ) were randomly performed to each patient. Respiratory mechanics were measured before (T0) and 5 min (T1), 30 min (T2) and 1 h (T3) after RM in all patients. Results After RM, recruited volume were significantly increased in four groups (P<0. 05). Compared with group A, C and D, patients in group B had a more increasing on recruited volume (P<0. 05). Platform pressure and peak pressure were decreased on T3 in group B, C and D (P>0. 05); platform pressure, peak pressure and static compliance of the lung were improved on T2 in group B (P <0. 05), but no difference in other three groups (P >0. 05). Conclusion RM can reduce airway pressure and platform pressure, increase recruited volume. Maintaining oxygen concentration with 40% during RM may be more beneficial in terms of respiratory mechanics and recruited volume.
