The effect of the hematocrit level on the safety of endoscopy and the prognosis for the patients with upper gastrointestinal hemorrhage
Abstract:Objective To explore the effect of hematocrit ( HCT ) level on the safety of endoscopy and the prognosis for the patients with upper gastrointestinal hemorrhage .Methods 920 patients were divided into the low HCT group ( HCT<30%, n=587 ) and high HCT group ( HCT≥30%, n=333 ) in our hospital .The clinical variables and outcomes , including cardiovascular events , intensive care unit transfer and death of the two groups were compared .Results 63.80%patients with upper gastrointestinal hemorrhage presented with a HCT less than 30%.Blood transfusions were more common in low HCT group (73.94% vs 24.02%, P <0.001).However, correlation between the amount of blood transfused and HCT level was poor .Eight cardiovascular events occurred during or after esophagogastroduodenoscopy , including 5 cases in the low HCT group and 3 cases in the high HCT group (P=0.29).2.38%patients in the low HCT group transfer into ICU and 2.70% patients in the high HCT group, and there was no significant difference between them (P=0.15).Moreover, there was no significant mortality difference between these two groups .Conclusion HCT shows nice clinical safety of endoscopy in the patients with upper gastrointestinal hemorrhage .These data suggest that it is not necessary to wait for the HCT to reach a certain level before endoscopy for the patients with upper gastrointestinal hemorrhage .Endoscopy which done as soon as possible helps to decrease the recurrent bleeding , increase the therapy efficiency and improve the prognosis for the patients with upper gastrointestinal hemorrhage .
